The Kebon Agung Malang Sugar Factory Polyclinic uses a health module made by Jack Roebijoso and the family doctor team of Universitas Brawijaya to help the medical team and patients in the process of managing diabetes. This health module contains directions and the patient’s medical record in managing diabetes. However, there are several problems in its application. The health module is still in the form of printed documents so it has the risk of being lost or damaged. If it is lost, it would be difficult for the doctor to do an analysis and diagnosis. Other than that, there are some cases where patients difficult to consult directly with the medical team because they have moved to another place. This research has a purpose to build an information system that can help manage patient’s personal information, manage patient’s medical records, and help patients and doctors to do a consultation through the system. In its development, the system is built on the mobile platform so the users can use the system wherever they want as long as it is connected to the internet network. The system development method is throw-away prototyping. Then the system is tested using validation testing methods to find out that the system has been meets user requirements and compatibility testing methods to find out that the system can run on the Android operating system above version five. The results show that the system is 100% valid and 100% compatible.
